A client’s record offers three ways to record something, and they differ in who sees it.
Internal notes. Only visible to you and your colleagues. Use them for what a colleague needs to know: an injury, an agreement about rates, a preference. Please note: a note is still personal data. If your client asks to see their data, you must be able to show it. So don’t write anything you wouldn’t read out to them.
Tasks. For yourself or a colleague, with a date if that helps. For example: send a new plan after their check-in. You can set a completed task back to open. Your client doesn’t see tasks.
Assignments. Your client does see these, on their home screen in the app. For example: weigh yourself on Monday morning and enter it in the app. Add a date if it needs to happen by a certain day. You can see whether it’s open or done.
Which to choose. If you need to do something, it’s a task. If your client needs to do something, it’s an assignment. If a colleague needs to know something, it’s a note.
